西洋流行歌壇的搞笑嘲諷天王非這位老兄「怪人艾爾」楊科維契莫屬。歷年來被他「開刀」過的他歌曲與歌手無數，連「雅虎」（Yahoo!）網站的風格都被他拿來開玩笑變成他的「楊虎」（Yankovic!）網站。瑪丹娜在80年代一炮而紅的＜宛若處女＞（"Like A Virgin"）又豈能逃過這位「宛若外科醫生」的艾爾的手術／劫數。
怪人艾爾喜歡用原歌曲的名字諧音去改寫歌詞，然後再將原歌手的音樂錄影帶拿來依自己的歌詞模仿拍攝一遍，加上他老兄的嗓音也有一定水準，笑果與效果都非同凡響。台灣出了個豬頭皮（朱約信）風格與他類似，只是在產量上絲毫無法望楊兄之項背。這首＜宛若外科醫師＞向來是艾爾迷的票選最佳艾爾嘲諷歌曲的前三名，另外知名的還有模仿麥可‧傑克森Michael Jacksond的名曲＜壞＞（"Bad"）而寫成的＜胖＞（"Fat"）。
歌曲仿照瑪丹娜對情人唱出她因愛而感到全新的處女情懷，但是內容卻是描述一位剛從醫學院勉強畢業的實習醫生，即使還沒有能力開刀，卻必須第一次拿起手術刀硬是給他開下去的情景。在音樂錄影帶中這位宛若外科醫生的艾爾身著手術服，一方面極盡搞笑之能事，將手術房當做魔術表演台，從病人切開的肚子裡「變」出絲巾、輪胎鋁圈，連兔子都抓出來，博得助手與護士們的齊聲鼓掌。另一方面，則是十分「宛若瑪丹娜」地將擔架推車當做瑪小姐音樂錄影帶中的威尼斯小船，同樣學瑪小姐坐在上頭露出肚臍（只是肚子上多了不少體毛），隨著擔架推車的行進搔首弄姿，相信連天后本人看了都會忍不住捧腹大笑，佩服艾爾的創意與搞笑功力。
